Chad first visited the 2016 Community Care Case Managers Conference. At that time Chad had just completed his initial NDIS plan.
He returns to speak about his NDIS experience over the past 2 years. Chad’s mother Wendy will also share a few thoughts and we will also hear from Loretta Martin about her journey as a carer.

A presentation and discussion on the NDIS and Health Interface Project in the Barwon Region. This was a partnership project between Deakin University Geelong, Barwon Health and G21.
A final report and a toolkit of resources was developed in 2017 which is now available for the health and community care settings.
